Refund/Return Policy:
Once enrolled you are committing to this plan for one full year and as such will not be able to terminate the coverage prior to completion of one continuous year of coverage. You have the opportunity to renew prior to the end of the coverage term in order to avoid any interruption of coverage.

If a member cancels his or her membership within the first 30 days after the effective date of enrollment in the plan, the member will receive a full refund, exclusive of the $20 processing fee, after written or electronic notification of cancellation is provided to Florida Dental Benefits (FDB).

The member will receive a full refund upon receipt by Florida Dental Benefits of the member's notification of cancellation. To ensure cancellation request is processed, Member must provide written notification by mail (801 Arthur Godfrey Road, Suite 300, Miami Beach, FL 33140) by email ([email protected]) . A full refund will be made within 30 days of receipt of the notice of cancellation. After 30 days of initial purchase, Florida Dental Benefits Refund Policy is as follows:

Delivery Timing/Method:
Florida Dental Benefits is offering two dental plans. Select the plan that's right for you and complete the Subscriber Application; you can enroll your spouse, domestic partner and/or children under age 26.

Make sure you indicate your plan choice on the application. Select your payment method, either credit card or check. Please note if you are paying by check you will need to make it out to: Florida Dental Benefits for the entire annual premium.

If you pay by credit card please complete the credit card recurring payment form. The credit card you provide will be charged the monthly premium every month for a 12 month period.
