How to Enroll:
Chose a plan that’s right for you. There are 2 plans to choose from, our Basic Option Dental Plan with Specialty Care and High Option Dental Plan with Specialty Care. Then choose a dentist from our network of 700+ participating Florida private practice dentists.

Choose the plan and payment method and complete the online Subscriber Application. You can enroll your spouse, domestic partner and/or children under age 26.
You can also print the application and fax it along with the credit card authorization form to the fax number below. If you are paying by check please mail your completed application along with your check made out for the annual premium to:

Once your application and payment is processed you will receive written notice of your acceptance and effective date of coverage. Enrollment in the Florida Dental Benefits plan is for a minimum of 12 consecutive months.
